Statement
The information
gathered from this form is used solely for pre-registration purposes
of the MCARC VE Team. With the exception of a telephone number,
everything asked on this form can already be gathered by call-sign
lookup on any of the Internet call servers.
This information
will be transmitted in plain text to our email address and we will try
to respond via email within 48 hours.
General
Information
All license
elements (Technician, General, and Amateur Extra) will be offered.
Pre-registration is
required only for special testing accommodations.
Applicants need to
bring the
original of
their amateur radio license (if applicable) to show, a photocopy of
the license to file, and photocopies of any other paperwork that will
be presented for element credit (i.e. CSCE). Applicants must also show
a government issued photo ID (no need to supply a photocopy of the
ID).
Successful
applicants must complete the
NCVEC form 605
which will also be available at the test session. This form requires
the applicant's Social Security Number (SSN) or the applicant's FCC
Registration Number (FRN). Applicants without a FRN may visit the
FCC website and apply for a FRN in
advance.
There is no fee for
the license exams.
